1. Download the Customer Information Loan form. You will need to get the free Adobe Acrobat Reader in order to use it, if you don't already have it.

• Check for $65 for Credit Report

• W-2's for the last two years

• One month's worth of recent pay stubs

• Two months of bank statements on account where source of funds are located.

• Self employed or commissioned employees will need to provide 2 years of tax returns.

2. Follow the simple instruction and then drop it off at your nearest office. Make sure you include the following documents with your packet.

3. A loan officer will review your documents and contact you with more information.
